Who Can Join
If you own a business in any of the following Florida and Georgia counties and the business is “active” on sunbiz.org in Florida or ecorp.sos.ga.gov in Georgia, you are eligible for membership.

Joining is Easy
When you join, we will ask for the information and documents below to confirm your identity and that your business is active.
All beneficial owners and individuals that will transact on the account(s) must be individually eligible for membership, and all personal accounts with VyStar must be in good standing.
The Certification of Beneficial Owner(s) form must be completed for all legal entities.
Any individual that will transact on the account(s) must sign the Business Membership application to authorize account opening. These individuals must also agree to be on the account.
A valid/current form of government issued ID must be provided for all beneficial owners and any individuals that wish to transact on the account(s).
Please note: A beneficial owner is considered any individual(s) who directly or indirectly owns 25% or more of the legal entity. At least one beneficial owner and at least one person with significant managerial control in the business (CEO, MGR, PRS, VP, TREAS, etc.) must be listed as signer(s) on the account.
How to Apply
You can apply for business membership at any VyStar branch.
Start by downloading the Business Application Checklist to ensure you have all the information you need. If you’d like, you can complete your Certification of Beneficial Owner(s) form before you come in.
A Business Services representative at the branch will witness the signing and dating of your application. If you have additional questions, please call us at 904-908-2292.
Sole Proprietorship
What you’ll need:
A copy of your current and valid driver’s license
Proof of your Federal Tax ID Number (if not using personal Social Security Number), evidenced by an IRS letter, business tax return document (from the past 12 months) or tax payment coupon
Fictitious name filing with State of Florida/Georgia, if using a DBA name
Make sure all signatures are provided on your Business Membership Application.
General Partnership/Limited Partnership
What you’ll need:
Copy of a partnership agreement that lists ownership percentages
Fictitious name filing, if applicable
Copy of a current and valid driver’s license for each partner
Proof of your Federal Tax ID Number, evidenced by an IRS letter, business tax return document or tax payment coupon
Verification of principal business address for the business
Completed Certification of Beneficial Owner(s) form
Make sure all signatures are provided on your Business Membership Application.
Limited Liability Company (LLC)
What you’ll need:
Copy of Articles of Organization, Latest Amendment, most recent Annual Report, Operating Agreement or K1 Tax Document (document must list current beneficial owners)
Copy of current and valid driver’s license for all beneficial owner(s)
Proof of your Federal Tax ID Number (if not using personal Social Security Number), evidenced by an IRS letter, business tax return document (from the past 12 months) or tax payment coupon
Active Fictitious Name Filing with LLC listed as owner, if using DBA
Verification of principal business address for the business
Completed Certification of Beneficial Owner(s) form
Make sure all signatures are provided on the Business Membership Application.
Corporation
What you’ll need:
Copy of Articles of Incorporation, latest Amendment, most recent Annual Report, Bylaws, K1 Tax Document, Corporate Resolution or Shareholder’s Agreement (document must list current Beneficial Owners)
Copy of current and valid driver’s license for all beneficial owner(s) and signers
Proof of Federal Tax ID Number, evidenced by an IRS letter, business tax return document (from the past 12 months) or tax payment coupon
Active Fictitious Name Filing with Corporation listed as owner, if using DBA
Verification of principal business address for the business
Completed Certification of Beneficial Owner(s) form
Make sure all signatures are provided on the Business Membership Application.
Clubs/Organizations/Non-Profit
What you’ll need:
Copy of Articles of Incorporation (if incorporated), latest Amendment, most recent Annual Report, Bylaws or Minutes stating the officers representing the organization
Copy of current and valid driver’s license for all signers
Proof of Federal Tax ID Number, evidenced by an IRS letter, business tax return document (from the past 12 months) or tax payment coupon. (If you are a small club operating under another group, a letter from the sponsoring organization authorizing use of an EIN is required.)
A letter from the organization that acknowledges the account opening, confirms the identity of the organization’s members authorized to open the accounts and signed by the authorized officers representing the organization
Verification of principal business address for the business
If incorporated, Section II completed of the Certification of Beneficial Owner(s) form detailing an Officer/Director listed in the provided documentation stating the officers representing the organization.
Make sure all signatures are provided on the Business Membership Application.
