How to Join VyStar
We appreciate your interest in VyStar Credit Union and welcome new members. To help you join VyStar and open your savings account, we have provided a printable NEW MEMBERSHIP APPLICATION / Account Card to fill out, print and return to us either by mail or in person.
By MAIL: If you plan to mail your Membership Application / Account Card to us, please be sure to include:
- Your completed, signed and notarized Membership Application / Account Card.
- A check or money order in the amount of $10 is required, which covers your $5 membership deposit into your Primary Savings Account and $5 membership fee to join VyStar Credit Union.
- In accordance with the USA PATRIOT Act, we are also required to obtain the following information when opening a new account by mail:
- Notarized copy of your driver's license or other valid government-issued identification.
- Notarized copy of a billing statement, such as a cable, telephone or utility company bill.

IN PERSON: Apply for membership at any VyStar Branch location. To save time, you can complete your Membership Application / Account Card before you come in but, do not sign and date it. A VyStar Member Relationship Specialist at the branch will witness the signing and dating of your application – we have free branch notary service. Also, please bring your driver’s license or other valid government-issued identification and a copy of a billing statement, such as a cable, telephone or utility company bill to prove current address/residency.
If you would like to drop off your Membership Application / Account Card at a VyStar Branch, the instructions "BY MAIL" will apply.
BY PHONE: If you would like to speak with a VyStar Representative about your Membership Application / Account Card over the telephone, call VyStar’s Call Center at (904) 777-6000, option 9 or (800) 445-6289, option 9.
TO OPEN AN ACCOUNT FOR A MINOR: When opening an account for a minor (under 18 years of age), you will need to have a certified copy or original of their birth certificate and social security card available, or a state identification card.
Note: In the future, if you would like to make a change or add another person to your account (for example, a spouse), please contact us for assistance. There is a form you will need to complete. We can also help you with any changes you would like to make to beneficiaries you currently have listed on your application.
